Please be aware that this GP practice is not
currently prescribing Mounjaro (Tirzepatide) for weight loss
Why?
Mid & South East Essex ICB has not commissioned this practice to prescribe Mounjaro for weight management purposes.
While some patients may have received Mounjaro under previous or historic arrangements, these are not being newly initiated
and only existing prescribing arrangements remain in place at this time.

Opting out of NHS Digital collecting your data (Type 1 Opt-out) – IMPORTANT PATIENT INFORMATION
The following link from NHS Digital explains why data is shared, it contains their transparency & privacy notices for doing so and also points to links where the patient can opt out if they want to: Opt out of sharing your health records - NHS (www.nhs.uk)
If you wish to register a Type 1 Opt-out with your GP practice before data sharing starts with NHS Digital, this should be done by returning the Type 1 Opt Out Form to your GP practice.
If you have previously registered a Type 1 Opt-out and you would like to withdraw this, you can also use the form to do this. You can send the form by post to The Surgery 1a Hawkesbury Road Canvey Island SS8 0EX or by email to cprccg.thesurgeryf81205@nhs.net
Once we receive your completed forms our reception staff will code the opt out accordingly.
