You are aware that COVID restrictions came to an end on the 19th July.
However, NHS England have advised that it is mandatory for all healthcare settings to adhere to current infection control guidelines.
This means that at the surgery we will still require our patients to wear masks, (unless you are exempt) use the hand sanitisers provided and we will still be following social distancing guidelines.

Opting out of NHS Digital collecting your data (Type 1 Opt-out) – IMPORTANT PATIENT INFORMATION

If you wish to register a Type 1 Opt-out with your GP practice before data sharing starts with NHS Digital, this should be done by returning this form Type 1 Opt Out Form to your GP practice.
There is now no urgency to process Type 1 opt-outs in order to get patients opted out before September. The cut-off date has now been deferred. We will keep you updated on timelines for when we expect the programme to go live.
If you have previously registered a Type 1 Opt-out and you would like to withdraw this, you can also use the form to do this. You can send the form by post to The Surgery 1a Hawkesbury Road Canvey Island SS8 0EX or by email to email@example.com
Once we receive your completed forms our reception staff will code the opt out accordingly.

For all urgent appointments, we ask that you undertake a symptom assessment through DoctorLink.
The symptom assessment will ask you a number of questions to help to identify your problem.
The outcome of your assessment will direct you to the right treatment, whether emergency care, your GP, nurse or self-care.
If your outcome indicates that an appointment is required at your GP practice once the request has been submitted, you will then be contacted by the Practice Team, to arrange an appointment within the identified timeframe (as outlined in the outcome of your symptom checker).
The information you provide will be shared with our clinical team (where appropriate) and will assist in providing robust information of your problem to the required clinical team member prior to your appointment.
